What subfloor preparation is needed for 24×24 tiles?

Large-format tiles (anything over 15 in on either side) require a very flat substrate. The Tile Council of North America (TCNA) specifies no more than 1/8 in variation in 10 ft for tiles up to 15 in; for tiles larger than 15 in the limit is 1/8 in in 10 ft (3 mm in 3 m).
